You can obtain a copy of your Driving Record from any Missouri Department of Revenue contract office, or you can request a copy of your Driving Record through mail.  The following requirements must be met:

• The completion of form DOR-4681 with your signature

• A Notary’s signature on form DOR-4681

• Cashier’s Check, Money Order or Personal Check payment of $7.00
  

To obtain a copy of someone else’s Driving Record, you must first have the person’s permission.  You can complete your request in person at any Missouri Department of Revenue contract office or by mail, and you must include the following:

• The name, date of birth and driver’s license number of the person whose Driving Record you are requesting

• Cashier’s Check, Money Order or Personal Check payment of $7.00

When requesting a copy of a Driving Record through mail, whether it is your own or another individual’s, your request and all applicable forms and information should be sent to:
